If you'd like to remove the top bar Apply button in the form, follow the steps below:

1. Access the Style Editor. Open a list item in either view or edit mode.
Click the gear icon at the top-right corner of the form, then select Styles.

2. Paste the following CSS into the Styles Editor section:

.ms-CommandBar .ms-Button--commandBar.is-disabled{
   display: none;
   }

Once applied, the Apply button on the top bar will no longer appear on your form.
